Kansas Healthwave

We are proud to be the Hays providers for the Kansas healthwave program. 

To serve you better please understand the following: 

We accept residents of Ellis County.  Residents of other counties must get a referral from their local healthwave dental provider, if you do not have a local provider contact our office for more information. Healthwave offers dental coverage for children under the age of 21 only.  We do not currently accept adults on healthwave. Dental services covered include most preventive, restorative and emergency care services. You must bring your insurance card to every appointment.  If you are not covered on the date of your appointment you will be responsible for fees for any services rendered. A parent or legal guardian must accompany a minor child to the initial appointment. We schedule by appointment only, all appointments require 24 hours notice for cancellation.  Failure to cancel with 24 hours notice and failure to show up to your scheduled appointment may cause us to dismiss you from our practice.
